Steps to Enrollment

Enrollment in INTERMACS will allow your facility to enter clinical and laboratory data for patients in end stage heart failure. Your participation in the registry will enable research to determine "best medical practices" with respect to the use of MCSDs in treatment of heart failure.

1. Enrollment is required to participate in the registry.
2. Three simple steps must be met to complete enrollment:
• Collect and enter demographic data about your facility
• Collect and enter demographic data about your personnel
• Review your enrollment application and submit
