INTERMACS Membership Information
Six steps to INTERMACS membership:
- Review the Administrative Training PowerPoint presentation and submit your completed post-test to INTERMACS Support.
Administrative training objectives:
- Participants will be familiar with the purpose and structure of INTERMACS.
- Participants will know enrollment and activation procedures for INTERMACS participation.
- Participants will be familiar with hospital membership criteria and the audit process.
- Participants will be familiar with INTERMACS Adverse Event Definitions.
- Participants will know how INTERMACS Support contact information for assistance with enrollment and data entry issues.
- Online Enrollment
- Submit documentation of IRB approval and copies of all IRB-approved informed consent forms (see Appendices B and C of the Manual of Operations) to INTERMACS Support.
- Submit completed Conflict Of Interest form(s) for each identified Principal Investigator and Co-PI (Appendix E of the Manual of Operations) to INTERMACS Support.
- Submit signed Participation Agreement (Appendix D of the Manual of Operations) to INTERMACS Support
*** steps 1-5 must be completed and documents submitted prior to practical training:
- Attend a practical Web-Based Data Entry Training session: Interactive demonstration and data entry tips will be provided via conference call and Microsoft Live Meeting. The trainer will review the application, Quality of Life and Neurocognitive assessments, and the Blood and Tissue repository protocol. Prior to training, participants should print the Users Guide as well as Appendices F, G and H for reference during the practical training session.
Practical Training Objectives:
- Participants will understand the flow of data within the database.
- Participants will be familiar with INTERMACS Inclusion/Exclusion criteria.
- Participants will able to access and complete data fields in INTERMACS.
- Participants will be familiar with the Quality of Life and Neurocognitive assessment tools.
- Participants will be familiar with the procedure for blood and tissue acquisition and documentation.
